Senior Services Programs
Assisted Living Waiver
The Assisted Living Medicaid Waiver program pays for services for eligible residents living in participating assisted living facilities (ALF) who would otherwise meet nursing home criteria. To be eligible, a person must meet financial and functional eligibility criteria and be 60 years of age or older. All individuals must be assessed by a Case Manager in order to qualify and/or receive services.
Case Management
Provides assistance in accessing and coordinating needed medical, educational, supportive or other services.
Caregiver Training/Support
Caregiver training/support service is designed to encourage the provision of day-to-day care for the client by the persons in his/her family and social circle.
Chore
Perform house or yard tasks including seasonal cleaning, essential yard work, lifting and moving, simple household repairs, pest control and household maintenance.
Companionship
Relieves loneliness and provides continuing social contact with the community by casual conversation, assistance with reading, writing letters, or entertaining games.

Consumable Medical Supplies
Provides supplies used by the client, caregiver, or service staff which are essential to adequately caring for the needs of the client.
Counseling
Advises and enables the older person and/or his/her family to resolve problems.
Depression Screening
The Senior Division provides depression screening through the Healthy Ideas (Identifying Depression, Empowering Activities for Seniors). The program includes; educational material, helpful interventions to alleviate depressive symptoms, and referral/linkages to medical help for more chronic depressive needs.
Education/Training
Provides formal or informal opportunities to acquire knowledge, experience, or skills. This includes individual or group events designed to increase awareness in areas like nutrition, crime and accident prevention, or to promote personal enrichment.
Emergency Alert Response
Provides a community based electronic monitoring system working with the telephone line to allow the calls for help when needed. It is monitored 24 hours per day at a central monitoring station.
Elder Helpline and Information/Referral

Health Support
Activities are designed to assist persons to secure and utilize necessary medical treatment. This may include; preventative, emergency and health maintenance services.
Home Delivered Meals
Provides a nutritious meal to an eligible, functionally impaired, homebound participant.
Homemaker Services
Provides a trained homemaker to accomplish specific home management duties including but not limited to; housekeeping, meal planning, meal preparation and storage, shopping, errands, and bill management.
Housing Improvement/Emergency Home Repair
Provides home repairs or alterations to the client’s home in instances where there are housing code violations or dangers that would prohibit the client from staying in their own home.
Legal Services
Legal assistance services are provided to assist clients to become aware of and protect their civil/legal rights through activities or direct intervention by attorneys or legal paraprofessionals.
Nutrition Counseling
Provides individualized or group advice and guidance to individuals who are at nutritional risk because of nutritional history, current dietary intake, medications or chronic illnesses. Options for improving the client’s nutritional status is provided by a registered, licensed dietician or other health professional functioning within their legal scope.
Nutrition Education
Presentations are designed to promote better health by providing accurate, informative and culturally sensitive instructions on nutrition, physical fitness or health (as it relates to nutrition). Nutrition education may be tailored to groups or individuals and is overseen by a dietician or individual of comparable expertise.
Personal Care
Provides assistance to functionally impaired elders with bathing, dressing, ambulation, housekeeping, supervision, emotional security, meals, and securing health care from appropriate sources.
Respite Care
Provides relief or rest from the constant/continued supervision, companionship, therapeutic and/or personal care of a functionally impaired older person for a specific period of time.
Risk Reduction
Combines either hands-on care or guidance to the caregiver and client to promote wellness and prevent premature institutionalization.
Transportation
Provides travel to and/or from providers of community agencies and resources. List of providers may include but are not limited to; supermarkets, doctors, laboratories, or nursing homes to visit a loved one. Read more…
